Important Notice

L&J Insurance Services, Inc. understands and shares your concerns about privacy. This notice describes the use and accessibility of non-public, personal information ("client information") which is collected from you in order to provide the best service possible.

L&J Insurance Services, Inc. protects your information - and your privacy.

* We do not share information about you or your policy unless authorized or otherwise lawfully regulated.
* We do not sell client information.
* Companies that do business on our behalf are expected to maintain and understand similar privacy policies.
* Personal files and client information are protected at all times.

Information We Collect
The information we have on file comes directly from you or is obtained from other sources (such as prior insurance carriers), with your consent. We also obtain documents of public record for our files. The application you fill out provides us with the information needed to best serve your interests. When outside information about you is required in order to establish a policy, the party disclosing the information must present a signed waiver indicating that you allow the transfer of that personal information to L&J Insurance Services. If you have not signed a waiver that specifically gives L&J Insurance Services, Inc. the right to have knowledge and/or possession of the information, we will not accept the information.

You are in control of your personal information at all times. If you wish to review the record we have on file regarding your account, you are welcome to do so. Please contact our offices to arrange such a meeting.

Information We Distribute
The only information about you that leaves our office is the application and supporting documentation necessary to obtain an offer for coverage. There is a waiver on this form, signed and dated by the applicant, which shows understanding and acceptance of the terms of sharing information with carriers in order to obtain medical malpractice insurance coverage. If your application is not signed, indicating that you understand this information will be shared, we cannot work on your behalf.

When coverage is confirmed, there is a certification sent to the practice and hospital you are affiliated with and other authorized requestors indicating proof of insurance. We must have a signed waiver on file before we can share this information which is vital to your career.

Why We Keep Information
We keep your information on file to best serve you. We may, without authorization but only as permitted or required by law, provide client information to persons or organizations inside and outside L&J Insurance Services, Inc. to fulfill a transaction you have requested, service your policy, detect or prevent fraud, participate in insurance support groups, or comply with lawful request from regulatory or law enforcement agencies.
